Patient Transition Notice
An Important Update on Our Next Chapter and Your Care
Announcement
It has been an incredible honor and privilege to serve as your healthcare partner. We are writing to share some important details about an upcoming transition for Dr. Burchell and our practice as we prepare for our next chapter.
Over the next few months, NorthStar Chiropractic will gradually decrease its weekly treatment days and hours. Our final week of seeing patients in the clinic will be September 28th, 2026.
Your health and continuous care remain our top priorities during this transition. Dr. Burchell will be available for patient treatment at a different clinic and location following this transition period. We want to ensure you have ample time to establish care with a new provider if necessary. We recommend exploring options through your insurance directory, checking with local healthcare networks, or contacting theColorado Chiropractic Association (CCA) Find a Doctor Tool for local provider referrals. Chiropractic Records Access
Your chiropractic records remain completely secure and confidential. In accordance with Colorado State Record-Keeping Rules, we will transfer copies of your file to your newly designated chiropractor upon receiving your signed authorization. To request a records transfer form, please email us at [email protected]. Please allow up to 30 days for processing. After the week of September 28th, 2026, your records will be securely stored by Jane Software Inc. and will remain accessible to you.
Packages and Memberships
We want to ensure a seamless and fair transition for all of our contract and package holders:
Package Holders: If you have an active multi-treatment package, you may submit a written request via email to[email protected] for a prorated refund based on your remaining, unused treatments.
NorthStar Choice Members: If you are on a monthly membership, you may submit a written request via email to [email protected] to conclude your membership early. All standard early-ending fees or cancellation charges will be fully waived.
